Most of the information is self explanatory. However some of the data does need further clarification. Review the sample while reading these instructions.

  • Enter your name, series, grade, position title, organizational element, and your supervisors name on the first row of the form. Section I is used to record your short-term and long-term goals. Review the section in this chapter on goal setting.
  • Section II is used to capture:
  • Section III is designed to capture formal training and schedules. You will also list the estimated cost for this training, projected and actual completion dates.
  • Signature blocks are included at the bottom of the form for the immediate supervisor and employee to sign. The Division Manager’s signature is optional.
